
Valuing People Now and Transition - Regional Events
These events are for people who have a lead within organisations for the development of best practice in transition for young people with learning disabilities. They have been designed to support Learning Disability Partnership Boards to deliver on the Valuing People Now objective, that all young people with learning disabilities will have person-centred transition plans by 2012, and that the plans focus on health, employment, housing and friendships and community inclusion. The events also address the priorities set by the Transition Support Programme and will be very relevant to people working to improve transition for the wider group of young people with SEN and disabilities.

Person-centred transition planning principles: how to make sure that young people have a strong voice and look forward to full and independent lives
This session will set out the principles of person-centred transition planning and the tools that have been used to ensure that transition review meetings enable young people and their families to have a strong voice and to say what is important to them now and in the future. There will be an emphasis on action planning from year 9, which will support young people to move into adulthood with jobs, friends, their own homes and to have choice and control of their lives.
This session will set the scene and will be particularly relevant to the person or people who have the lead for transition in the local area as well as practitioners. This could include:
- The TSP Lead
- Children's Services
- The chair of the local transition strategy group
- The learning disability partnership board lead for transition
- Schools
- Colleges
- Person centred planning leads

Planning for Good Health
The Government published Moving on Well in 2008 to highlight the importance of good transition planning for young people with health needs. Young people are discharged from specialist paediatric health services between 16 and 18. This is often the end of specialist involvement. It is important that all young people with learning disabilities plan for good health as they move into adulthood, but it is particularly important for young people who have lifelong conditions and those who are eligible for "continuing healthcare funding". This session will look at how to link person-centred transition planning with health action planning and other ways of supporting young people and their families.
This session will be relevant to any of the people listed in the introductory session but in addition is for:
- The lead for young people with SEN, disabilities and complex needs in the PCT
- The Learning Disability Partnership Board lead for health
- The lead for health action planning and health facilitation

Planning for Jobs and Careers
The publication of Valuing Employment Now, the lessons learned from the Getting a Life programme have shown the need to support young people with learning disabilities to plan for their careers, to get work experience and part-time jobs. We know that young people need a person-centred approach, which in relation to employment is called "customised supported employment". Participants will find out about the key elements of customised supported employment, and how customised supported employment can help young people to get jobs.
This session will be relevant to any of the people listed in the introductory session but in addition is for:

Friends and Relationships
Perhaps the most important aspect of life is having people around who love and care about us. People with learning disabilities are often not supported to make and keep friends or to develop adult relationships, and consequently they report feelings of loneliness and isolation. The young people in the Getting a Life programme have told us that having friends, boyfriends and girlfriends is very important to them but they need more help to achieve this. This session will focus on how transition planning can ensure that young people are included in community life, have circles of friends and circles of support and receive the information and help they need to develop adult relationships

- People with experience developing and supporting circles of friends and circles of support

Transition planning, personalisation and support planning
This session will enable participants to bring together transition planning and personalisation. Young people and their families need to have clear information about individual budgets from year 9, so that they can plan for the future knowing what resources they will have and what support they can expect. We need to blend transition planning, customised supported employment and support planning and to use the associated funding streams so that they are part of one seamless process and work together, rather than operating as a number of parallel activities.

- Personalisation Leads
- Organisations developing brokerage support
- People who are working to develop support plans
